Enhanced Care Management

Gateways Hospital provides Enhanced Care Management (ECM) services to children, adolescents, and adults eligible for the Medi-Cal benefit. ECM is a free, concierge level personalized health care support service for individuals who have struggled to manage their health, work, or housing. This statewide Medi-Cal benefit connects people with the resources they need to maintain stability and improve their quality of life. By simplifying complex healthcare and social service systems, ECM guides individuals through personalized care navigation, helping them achieve their goals and live more stable, supported lives.

Core Services

Enhanced Care Management provides members with in-person Lead Care Managers (LCM) and Community Health Workers (CHW) who meet with clients, families, and caregivers where they are in the community. Below are some of the personalized services Gateways provides through ECM:

  • Personalized coordination of care for mental health, substance use treatment, medical needs, housing, and more.
  • Food and basic needs support, including help finding food banks and meals on wheels.
  • Short- and long-term housing assistance.
  • Transportation assistance to get to appointments.
  • Employment and education support.
  • Help navigating the justice system, including re-entry into the community, probation, parole, and court needs.
  • Health education and support.
  • Help transitioning between levels of care.
  • Referrals for any other social supports.

Participation Eligibility

Individuals and families may qualify for this free service if they have Medi-Cal and experience any of the following challenges:

    • Experiencing or at risk of homelessness
    • At risk of avoidable hospitalization or transitioning out of the hospital
    • Serious mental illness and/or substance use disorders
    • Transitioning from incarceration
    • At risk of placement in long-term care inpatient care
    • Transitioning out of inpatient nursing center
    • Involved in the child welfare system
